Alumni (graduated more than 5 years), or my Diploma is Lost or Damaged
ORDERING A DIPLOMA
If your graduation date exceeds five years, you must file the Application for Replacement of Diploma even if the original diploma had gone unclaimed. Replacement diplomas will bear the signatures of the current University and State officials. The statement "This diploma was reissued" and the month and year it was reissued will appear in smaller print toward the bottom of the replacement diploma.
The Office of the Registrar places diploma orders once a month at the beginning of each month. Replacement applications received after the beginning of the month will not be placed until the following month. It typically takes 45-60 days to process a replacement diploma order.
Diploma Replacement Fees:
$60 for Undergraduate and Graduate Diplomas
$75 for Doctor of Optometry and Law diplomas
Check or Money order should be payable to the UC Regents
Mailing fee is included in the cost of the replacement of diploma, including those sent to an international address.

I HAVE ALREADY GRADUATED AND WOULD LIKE TO CHANGE MY NAME ON THE DIPLOMA
If you have already graduated and would like to change the name on the diploma, you must document the legal name change. For a list of permissible documents, refer to Name Change Policy. Two forms are necessary for this type of transaction: The Petition for Change of Name After Awarding of Degree and the Application for Replacement of Diploma. Both applications forms, fees, supporting document(s), and the original diploma must be submitted on one envelope, addressed to:
