If you have graduated within the last five years and have not yet claimed your diploma and would like to have it mailed, download the Diploma Request Form. Mail your completed form. There is no fee for this service.

DIPLOMA PICK-UP
You can pick up your diploma at the Office of the Registrar, 128 Sproul Hall, Monday through Friday, 9 a.m. to 12 Noon and 1p.m. to 4 p.m.. There is no charge for picking up the diploma, we do require that you show photo I.D.

AUTHORIZING ANOTHER PERSON TO PICK UP DIPLOMA
In order to have someone else pick up your diploma, you must issue that person a signed note of authorization. It does not have to be formal; a handwritten note will suffice. Please include the following in your signed note:
# Your full name as it appears in University records
# The name of the person authorized to pick up your diploma
# Your college and major (example: L & S, MCB)
# The degree earned (example: BA, MA)
# The term and year the degree was awarded (example: fall 2010)
# Your signature
The person picking up the diploma must present a signed note from you and a photo ID to Office of the Registrar, 128 Sproul Hall, Monday through Friday, 9 a.m. to 12 Noon and 1p.m. to 4 p.m.. You may also fax the authorization directly to 510.643.9818.
CHANGE OF NAME ON A PREVIOUSLY ISSUED DIPLOMA
